Appointments Are Required  

1. Read these steps prior to making an online appointment

   • Appointment must be made in the name of the taxpayer. Only one appointment needed if married filing jointly. If someone else is making an appointment, enter that person's name and phone number in the "Notes to Counselor" box.

   • A separate appointment is required for each tax return, including returns for previous years. If you need a return for a prior year, indicate in the "Notes to Counselor" box what year (i.e.-TY2021)

   • Double check spelling of name, phone number, and email address for accuracy

   • If an email address is provided, the taxpayer will receive an email appointment confirmation and  2 email appointment reminders

2. An Intake Booklet must be completed before we can prepare your return. You can download this Intake Booklet and complete it at home prior to your appointment. Alternatively, a packet of forms may be picked up in advance at Oak Street Health. Ask for the AARP Tax-Aide packet of forms at the front desk.

5. Please read these arrival instructions prior to your appointment

   • Taxpayer who feels ill or has COVID-19 symptoms should not enter the building; we will give priority to rescheduling your appointment.

   • If you have not completed your paperwork in advance, arrive 15 minutes before your appointment to complete it.

   • If you are married and will be filing jointly, your spouse must be with you for us to file your return electronically (eFile), unless you have a Tax Power of Attorney. If your spouse is not present, we can prepare a paper return for you to mail to the IRS.

   • Only taxpayer (and spouse or Legal representative with Tax Power of Attorney)  named in the appointment reservation will be allowed in the tax preparation area of the building.

   • Please cancel your appointment if you no longer need it so that someone else may take your place.

   • Get help with making, cancelling, rescheduling, or looking up an appointment on the Appointment Help page. 

6. When you make an appointment you must complete all required information and select "Confirm Information" for your appointment reservation to be saved

This is a medical clinic. Masks are required.
