Appointments Are Required
Read these steps prior to making an online appointment
Appointment must be made in the name of the taxpayer. Only one appointment needed if married filing jointly. If someone else is making an appointment, enter that person's name and phone number in the "Notes to Counselor" box.
A separate appointment is required for each tax return, including returns for previous years. If you need a return for a prior year, indicate in the "Notes to Counselor" box what year (i.e.-TY2021)
Double check spelling of name, phone number, and email address for accuracy
If an email address is provided, the taxpayer will receive an email appointment confirmation and 2 email appointment reminders
An Intake Booklet must be completed before we can prepare your return. You can download this Intake Booklet and complete it at home prior to your appointment. Alternatively, a packet of forms may be picked up in advance at Oak Street Health. Ask for the AARP Tax-Aide packet of forms at the front desk.
