How to make an appointment

Select a location where you want to make an appointment

Read the instructions on the page carefully

Click Make an Appointment 

Select a date. If no appointments are available that date, nearest dates will be shown. Select a time (it will turn grey). Then click Make Appointment.

Complete the form. Don't bother with the "How did you book your appointment" box. Look entries over carefully to be sure your name is spelled correctly. Also verify that your phone number and (optional) email address are correct. When satisfied, click Review Appointment. 

Finally, click the Confirm Information to complete the appointment reservation
